Welcome, and thank you for choosing Nov Dental for your care! We are happy to welcome new patients into our Glendale office. Below, you’ll find everything you need before your first visit, including new patient forms for both children and adults, plus an overview of the financing and payment options available to you.

Patient Forms

Before your first appointment, please fill out the new patient forms below. Completing them ahead of time helps your visit run smoothly and lets us make the most of our time together.

When you arrive, a member of our team will greet you and bring you back to meet one of our hygienists and Dr. Oh. From there, you can share your concerns and your goals for your smile, and Dr. Oh will walk you through the treatment options that fit your needs.

Financial Options

Our office accepts most PPO dental plans, and our team is glad to help you understand your coverage. We also accept Visa, MasterCard, American Express, and Discover.

If you don’t have insurance, that’s not a barrier to getting the care you need. We offer financing through CareCredit, so you can spread out the cost of treatment in a way that fits your budget.

In House Membership Plan

We know not everyone has dental insurance and that does not have to get in the way of getting proper dental care for those without insurance. We offer an in-house membership plan, which can help you receive quality dental care at discounted fees.
