We are providing essential forms to be filled out for new patients, both for children and adults, along with variety of financial options to consider for your care.

Prior to your first visit, please visit the section below and complete the online new patient forms.
This will allow us to make the most of your time in the dental office. When you first arrive, one of our team members will walk you through the dental office to the operatory where you will be seen by one of our dedicated hygienists and Dr. Oh.
During the visit, you will be able to express your chief concerns and dental goals with Dr. Oh and receive recommendations for treatment plans according to your need.

In House Membership Plan

We know not everyone has dental insurance and that does not have to get in the way of getting proper dental care for those without insurance. We offer an in-house membership plan, which can help you receive quality dental care at discounted fees.
